ARMY COUNSELING EXAMPLES

RANK Lastname, as your immediate supervisor, I am obligated to take this opportunity to ensure proper guidance for your future in D Company, X/XX Infantry. This counseling is purely for professional growth and to ensure proper soldierly conduct on and off duty; we will discuss the following areas:
I. UNIT MISSION VII. DUTY HOURS/WORK SCHEDULE
II. CHAIN OF COMMAND VIII. MEDICAL CONCERNS
III. PHYSICAL FITNESS IX. PERSONAL APPEARANCE
IV. DUTY PERFORMANCE AND CONDUCT X. SELF-DEVELOPMENT
V. DUTY DESCRIPTION –
VI. FAMILY SITUATION/CONCERNS –
I. UNIT MISSION – Summerize the month you are counseling him on and what is comming up next month, the commanders intent, and the overall goal the Company and Battalion is working towards (ie., JRTC, Afghanistan, etc.)
II. CHAIN OF COMMAND – Any changes that have happened or will happen and how it will effect them.
III. PHYSICAL FITNESS – How they did last month and goals you want them to strive for next month, set bench marks.
IV. DUTY PERFORMANCE AND CONDUCT – How they did as soldiers, give three sustains and three improves.
V. DUTY DESCRIPTION – What their job is and what they need to work towards.
VI. FAMILY SITUATION/CONCERNS – Anything you know of or they have brought up in the last month and what was done to make it better or improve the situation (ie., Grandfather died and they went home, etc.)
VII. DUTY HOURS/WORK SCHEDULE – What it was and what it will be next month.
VIII. MEDICAL CONCERNS – Sickcall, profiles, etc and what you have done to work to get them better.
IX. PERSONAL APPEARANCE – How did they look? Was it professional? Did it look like they needed to improve?
X. SELF-DEVELOPMENT – Are they enrolled in school? Are they taking classes? Do they know about the $4,500 they can use? Do they know they can get a BA in the 4 years they are in? How much more can they make if they get out with a BA v.s. a job without?

Have them initial "I Agree" and sign/date below. Have them write what they will do in this box at the end of your session.
